I do not understand this joke?

You didn't hear the whole joke. After the doctor says the thing about pizza, pancakes, and pita bread, the man asks "Will it cure me?" and the doctor replies, "No, it's the only food that will fit under the door." Stupid joke, but it's implying that the man is so riddled with disease that they don't want to let him out.
